I authorize AZOVA to charge my credit card for the AZOVA Home Sleep Test. I understand that my information will be saved on file for future transactions.

The AZOVA Home Sleep Test kit must be returned after you complete your test. You will receive a free shipping label for the return. You must take your test and drop it off at a UPS Store (not drop box) WITHIN ONE WEEK of receiving it.

I authorize AZOVA to charge my credit card $650 if I do not return my kit to UPS within seven days of receipt, or if I return my kit with damaged or missing pieces.
