Step 2:

We’ll ship your HEALTHBOX Virtual Preventive Exam to your door

• You must complete your exam within 30 days of receiving it.
• Your kit includes a blood pressure cuff, measuring tape, and the Comprehensive Wellness Test. This test will check the following:

HbA1c, eAG, Creatinine, eGFR, AST, ALT, Cholesterol, HDL, LDL, LDL/HDL Ratio, Triglycerides, Cholesterol/HDL Ratio, VLDL, TSH, hsCRP, and Vitamin D.

• If your organization offers it, you may also receive:
  • At-home colon cancer FIT test (for adults 45+)
  • At-home prostate/PSA test (for men 55+)
• Once your results are in, you will complete a comprehensive health assessment and schedule your virtual preventive exam with your Whole Health primary care provider.
• Your provider will review your lab test results and health assessment data and create a plan for your health.
• You will receive your personalized Health Vitality Index Report including your health risks, chronic conditions, recommended health screenings and vaccinations. Your Whole Health provider will help you to create a long term plan for your health and then we’ll help you to get there!
