What is the AZOVA Whole Health Membership for employers?

The Whole Health Membership is an all in one health and wellness solution for adults and children. In addition to Integrative adult and pediatric Primary Care, the membership includes access to preventive health screenings, at-home lab testing (for adults), Core Four health coaching (for adults), chronic condition management, and more. In addition, when your organization joins AZOVA Whole Health, your members have access to a connected ecosystem of specialty care providers including dermatology, psychiatry, therapy, sleep medicine with home sleep testing and DME services, clinical pharmacists, functional medicine providers, functional nutrution providers, maternal care doulas and lactation consultants and more.

Each member will choose an AZOVA Whole Health primary care provider as their care team leader. If a member already has a primary care provider, that’s okay too! Their AZOVA Whole Health provider will still collaborate and communicate with their existing primary care and their health coach to make sure they get everything they need. Members can always see their AZOVA Whole Health provider as needed for chronic condition care, prescription refills and more.

Who is the AZOVA Whole Health Membership program for?

This program is designed for adults and children who want a convenient, prevention-focused, and affordable way to manage their health.

What if some of my members already have a primary care provider?

Yes. If a member already has a primary care provider, they will still choose an AZOVA Whole Health primary care provider who be their care team leader. Their AZOVA Whole Health provider will collaborate with their existing primary care provider and their health coach. Their Whole Health provider is always available for chronic condition care, prescription refills and more as needed.

How does the AZOVA Whole Health Membership differ for adults and children?

Adults (ages 18-64) have access to the full suite of membership services. Pediatric members (up to 17 years old) have access to board-certified pediatric providers as well as dermatologists, psychiatrists and therapists (ages 12+ in states where available) but will not receive the HEALTHBOX Virtual Preventive Exam (must be 18+). Our annual Pediatric Wellness Visit is only available to children ages 2+.

Can members use insurance to pay for services that they receive through Whole Health?

We accept some health plans for some of our specialty services. If we do not accept your health plan, we can submit an out of network claim. Some of our specialty services such as pharmacy clinical services, functional medicine are not covered by insurance and we provide a member’s only discount for these services.

How can I determine if you accept my insurance for specialty services that are not included in the membership fee?

To see if we are in-network with your plan, go through the appointment registration process, choose a provider and then our system will show you if that provider is in-network or out-of-network with your plan. We will submit a claim to your plan regardless of if we are in-network or out-of-network. We will make every effort to determine what your financial responsibility will be for the visit and may ask you to pay for your portion of the visit fee prior to your visit with an AZOVA provider.

Do you offer in-clinic visits?

We offer virtual visits in all 50 states and we offer in-clinic visits for members who come to our Alpine, Utah location.

What is a Care Navigator?

A Care Navigator is a member of your care team who helps you get the most out of your membership. They can help you connect with a provider, schedule appointments, help with labs and prescriptions, help with referrals, and answer any questions you may have.

How do members contact a Care Navigator?

Members can access their Care Navigator 24/7 through in-app and text messaging. Members simply log into the AZOVA app and send a message to “azovatriage” and they will be taken care of!

What is Core Four Health Coaching?

Core Four Health Coaching is a program rooted in functional medicine that focuses on four key areas of health and wellness: a) exercise and movement, b) nutrition, c) sleep, and d) emotional wellness and resilience. The health coach will work with your members to develop a personalized plan to help you improve health and manage chronic disease. in each of these areas.

How do members access the no-cost virtual urgent care services?

Our virtual urgent care services are available 24/7/365 from the member’s dashboard. To access the service, complete the membership enrollment and navigate to the membership dashboard. Members can register for virtual urgent care at any time from anywhere.
