Have the following information ready before you begin onboarding: This onboarding setup will take approximately 20 minutes to complete when you have all the items listed below ready. Please plan on an extra minute or two for each additional location that you add. We recommend you to complete the onboarding on a desktop or laptop computer.
Have the following information ready before you begin the onboarding process:
- Your pharmacy or facility NPI number.
- Your personal NPI number.
- Your state license numbers and expiration dates.
- Address and hours of operation for each location: Please have the address and office hours and hours of availability for test collection services for each location.
- Your malpractice certificate and expiration date (you will need to upload a photo of the certificate for credentialing purposes).
- Your ACH information (this is how AZOVA will pay you). AZOVA will provide payment to you for each test that you administer per the contracted rates for this program. The contract will be presented during the onboarding. We will pay you once monthly via ACH for each service that you provide.
- Credit card information. You will be asked to enter a credit card at the beginning of the onboarding. If you are a pharmacy, there is a $10/yr or $1/mo fee to join the network as it also includes membership in the full digital pharmacy network. All others have no cost associated with membership and your card will not be charged unless you choose to purchase additional software for your own use.
