If you purchased your AZOVA product, including HEALTHBOX™, from an authorized retailer, please speak with a representative from the original place of purchase.
If you purchased your AZOVA product, including HEALTHBOX™, directly from AZOVA, please email our customer support team at support@azovahealth.com with your order number for immediate assistance if you have questions regarding your order.
Non-Refundable and Non-Returnable Items
Due to the nature of our products and services, they are non-refundable. We apologize for any inconvenience. If you received your product and do not wish to use it, please dispose of the kit. Kits are non-returnable.
Damaged or Defective Kits
Please inspect your order upon receipt and contact us immediately at support@azovahealth.com if the kit appears to be damaged, defective, or if you receive the wrong item. If the lab is unable to process your order due to insufficient collection, please contact support@azovahealth.com.
Cancellations
Once your order has shipped, we are unable to provide a refund. If you submitted an order and have questions regarding your shipment, please contact customer support at support@azovahealth.com.
Proctoring Kits
If your order was for a kit that includes proctoring services for drug testing and the proctoring kit was damaged or defective, please contact the ordering employer for next steps.
If your order was for a kit that includes proctoring services for a covid test and the proctoring kit was damaged or defective, please contact AZOVA customer service at support@azovahealth.com.
