Next Steps

1. Register with AZOVA to activate your sleep care benefits.
2. Upload a copy of your prescription. If you do not have a copy, ask your prescribing provider to fax it to us at (801) 769-1287.
3. Once we receive your prescription, we’ll validate your order and ship the AZOVA Home Sleep Test to you.
4. Take your sleep test at home from the comfort of your own bed. You’ll receive a Sleep Data Insights Report in the morning.
5. Mail the kit back to us using the free shipping label. We’ll send you a link to book a virtual care appointment with a sleep care provider.
6. Get a sleep telemedicine visit and a personalized treatment plan, including prescriptions for CPAP or medications as indicated.
