Create your account and be sure to add your name and date of birth exactly as you provided it when you scheduled your COVID vaccine appointment. You will also be asked to provide the date and location where you received your vaccination and a photo of your government issued photo ID and vaccination card. If you do not provide your photo ID, we cannot confirm your vaccination record.
Complete the process for each person whose record needs to be confirmed. If you are an adult, you must create you own account and submit a request on your own behalf. If you are requesting confirmation on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. You can get as many records confirmed for as many dependents as needed.
