Please create your account and add your name and date of birth exactly as you provided it when you scheduled your COVID vaccine. You will also be asked to provide the date and location where you received your vaccination and a photo of your government issued photo ID and vaccination card. If you do not provide your photo ID, we cannot create your vaccination record.
Complete the process for each person whose record needs to be created. If you are an adult, you must create your own account and create a record on your own behalf. If you need to create an electronic record for a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. You can create a record for as many dependents as needed.
