Get Your Vaccination Record Verified and Get a Health Pass

This verification services is currently available only to Utah residents or those who received their vaccine in the State of Utah. If you are not a Utah resident or did not receive your vaccine in Utah, we will not be able to verify your records.

Please create your account and add your name and date of birth exactly as you provided it when you scheduled your COVID vaccine appointment. You will also be asked to provide the date and location where you received your vaccination and a photo of your government issued photo ID and vaccination card. If you do not provide your photo ID, we cannot provide you with a verified vaccination record.

Complete a record verification request for each person whose record needs to be verified. If you are an adult, you must create your own account and submit a request on your own behalf. If you are requesting the service on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. You can submit a request for as many dependents as needed.
