Step 1: Complete the Verification Request

Complete a vaccination record verification request through AZOVA. If your vaccinations were reported by your vaccination provider in the state of Utah, we will be able to verify them for you. If your vaccinations were not reported by your vaccination provider to the state of Utah or if you received vaccinations outside of the state of Utah, we currently cannot verify them. If you would like to verify your record outside of the state of Utah, please notify your local immunization registry and let them know you would like to receive your records through AZOVA. AZOVA is happy to integrate with every immunization registry to provide this service to you.
