You will be asked to provide your name, address and date of birth exactly as you provided it when you scheduled your vaccine. You will also be asked to provide the date, time and location where you received your vaccination and a photo of your government issued photo ID and vaccination card. If you do not provide your photo ID, we cannot validate your vaccination record.
Complete a record validation request for each person whose record needs to be validated. If you are an adult, you must create your own account and submit a request on your own behalf. If you are requesting validation on behalf of a dependent, please create your own account and then add your dependent on the screen that asks who is the patient. You can submit a request for as many dependents as needed.
