Schedule a video proctoring session for most rapid tests. You will receive a lab report and a Simple Health Pass Credential ID. Once you receive your result, enter your Credential ID where needed for entry to facilities, events and travel destinations.

How it Works

img STEP 1

Schedule your video observation call within the timeframe that you need to take your test. Schedule a separate appointment for each person who will be taking a test. All members of your group can join the call together, but each person must have their own appointment registration.

At the time of your video call, you’ll be connected to a proctor who will guide you through the testing process for your test. Open your test kit only after you are in the video call with your test proctor.

img STEP 3

Once your test results are ready, AZOVA will dynamically create a COVID Credential™ with your validated results. You’ll receive an SMS and an email link to view your results. You can share your results with anyone who needs them directly from your COVID Credentials via QR code, Credential ID or PDF.

Note: If you are scheduling an appointment on behalf of a dependent, first create your own account and then add your dependent's information on the patient information screen.
