You will be asked to upload a photo of the front and back of your insurance card and to enter the subscriber name exactly as it is spelled on your insurance card and the member ID of the person for whom you are requesting vouchers.
AZOVA will verify that the member is eligible to receive vouchers for free COVID test kits.
