Step 2: Receive Your Verified Electronic Record

AZOVA will manage your request and will attempt to locate and verify your record for you. We will provide a digital verification of your vaccination records that are available in the state of Utah. We can verify only records that have been reported by your healthcare provider to the state of Utah and therefore, we may only be able to provide verification for one or more doses. If we are unable to locate and verify ANY records, you will receive a full refund.
